Comparative Outcomes of Pectoralis Major Myocutaneous Flap Reconstruction for Cheek Defects in Oral Squamous Cell Carcinoma:A Gender-Based Study

Background: Reconstruction of oral cavity defects following ablative surgery for Oral Squamous Cell Carcinoma (OSCC) is essential for restoring function and aesthetics. The Pectoralis Major Myocutaneous (PMMC) flap remains a widely used technique, especially in resource-limited settings. However, postoperative outcomes may vary between male and female patients due to anatomical differences. To compare postoperative outcomes and complications of PMMC flap reconstruction between male and female OSCC patients. Materials and methods: A comparative observational study was conducted in the Department of Oral and Maxillofacial Surgery at Dhaka Dental College Hospital, Dhaka on January 2025 to January 2026. Sixty patients undergoing PMMC flap reconstruction for cheek defects were included. Among them, 30 were male and 30 were female patients. Sociodemographic characteristics, habits, comorbidities, surgical factors and postoperative complications were analyzed. Statistical analysis was performed using chi-square test and logistic regression. Significance was set at p ≤0.05. Results: Major complications occurred in 26.7% of females compared with 13.3% of males (p=0.044). Minor complications were also higher among females (43.3%) compared with males (26.7%) (p=0.040). Diabetes mellitus and female gender were significant predictors of postoperative complications. Conclusion: PMMC flap reconstruction remains a reliable method for cheek defect reconstruction; however, female patients show higher complication rates compared with males. Careful surgical planning and perioperative management are recommended to reduce complications in female patients. Chatt Maa Shi Hosp Med Coll J; Vol.25 (2); July 2026; Page 75-79
