CMV Associated CD4+ TEMRA Cell Expansion and Vaccine Responsiveness in Older Nursing Home Residents

Abstract Older adults with advanced frailty exhibit strikingly heterogeneous immune responses. In 86 nursing home residents, clinical characteristics and viral exposure were not associated with longitudinal humoral responses to SARS-CoV-2 vaccination. Rather, expansion of immunosenescent CD4 + T EMRA cells was robustly associated with poorer vaccine immunogenicity. Whilst CMV serostatus was not associated with vaccine response, it was strongly linked to CD4 + T EMRA cell expansion, implicating CMV-associated immune remodelling in vaccine response heterogeneity.
