Shadow AI. When Users Move Faster Than the Institution
Physicians increasingly use generative AI without institutional approval. This essay interprets such shadow AI less as individual misconduct than as the result of an unusually large speed gap between technological adoption and institutional integration, that is, a reversal of the direction of diffusion from user to institution. Besides time pressure, the pull of the technology itself is discussed as an explanation. A blanket ban without an alternative may push use into areas that are hard to control; this relationship is a hypothesis. Data risk, decision risk and epistemic risk are distinguished. The institutional answer proposed is an internal alternative, visibility and competence, with the caveat that visibility is not yet control. The essay draws on surveys, peer-reviewed studies and regulatory sources up to October 2026.
