Rethinking Angina Pectoris—Toward Conceptual and Diagnostic Clarity
The current definitions, explanations, and clinical classification of angina pectoris, particularly the traditional definitions of "typical" and “atypical” angina or chest pain and the stable versus unstable dichotomy, contains conceptual inconsistencies that impair understanding concepts, diagnostic clarity and clinical decision-making. This article challenges several long-standing assumptions about angina, especially regarding the "typical angina" construct and unstable angina definition. We argue for a redefinition grounded in pathophysiology of ischemia and clinical logic rather than rigid historical formulas. These criticisms carry important implications for both education and practice.
