TARGETING THE HEMATOPOIETIC AND HEMOSTATIC AXIS: MECHANISTIC INSIGHTS, TRANSLATIONAL THERAPEUTICS AND FUTURE PERSPECTIVES IN COAGULATION AND HEMATOLOGIC DISORDERS-A COMPREHENSIVE REVIEW

Drugs acting on the blood and hematopoietic system form a fundamental class of therapeutics crucial for managing diverse hematological disorders. Disruption in blood cellular dynamics or hemostasis leads to severe clinical conditions including anemias, thrombosis, acute ischemic events, and hematological malignancies, which heavily compromise patient outcomes.[1] The aims of these review is to systematically synthesize the pharmacological mechanisms of therapeutic agents targeting blood components and bone marrow hematopoiesis. The primary objectives are to evaluate the clinical efficacy of hematinics, recombinant hematopoietic growth factors, antithrombotic therapies, and immunomodulatory agents in restoring normal physiological functions.[2] The major purpose of this study is to highlight how targeted drug interventions address cellular and chemical deficits within the hematological network—ranging from nutrient deficiencies and impaired lineage differentiation to dysregulated coagulation and pathological platelet aggregation.A narrative synthesis of literature was conducted, evaluating established pharmacotherapeutic strategies and clinical guidelines.[3] The focus encompasses therapeutic pathways including erythropoiesis restoration (via iron, cyanocobalamin, and folic acid), recombinant lineage stimulation (erythropoietin and G-CSF), platelet inhibition (aspirin, clopidogrel), targeted fibrinolysis (streptokinase, alteplase), and cytotoxic immunomodulation. While existing therapies significantly improve patient survival, challenges such as major bleeding risks, hypersensitivity, adverse reactions, and drug resistance persist. Advances in biotechnology, precision targeted therapies, and genetic engineering hold great potential to refine drug specificity, minimize adverse effects, and usher in personalized hematological medicine.[4]
