Aggressive fibromatosis of the buccal region: a rare case report

Background Aggressive fibromatosis (AF), also known as desmoid-type fibromatosis (DF), is a rare soft-tissue tumor that arises from deep musculoaponeurotic structures and is characterized by infiltrative local growth and a high rate of local recurrence. The abdomen is the most common primary site, and head and neck lesions account for fewer than one-quarter of all cases; within this region, the mandible is the most frequently involved site, (38%). Wide resection is the primary treatment in 74% of reported cases. Local recurrence, rather than distant metastasis, is the most common adverse outcome, occurring in 16% of patients. Case report A 23-year-old man presented to our hospital with a painless mass in the right buccal region that had been present for one month. He reported no pain, numbness, or tenderness and had received no previous treatment. Treatment The patient underwent local extended resection of the right buccal lesion, followed by immediate reconstruction of the surgical defect with a pedicled buccal fat pad flap. Conclusion Local extended resection followed by immediate reconstruction with a pedicled buccal fat pad flap achieved a satisfactory outcome, with no evidence of recurrence during 10 months of postoperative follow-up. Long-term surveillance is ongoing.
