Uniportal endoscopic treatment in piriformis syndrome with gluteus medius calcific tendinitis: a case report
Introduction: Piriformis syndrome is a challenging cause of sciatic nerve entrapment. Endoscopic surgery is an option for persistent symptoms that are refractory to conservative treatment. Case presentation: A 47-year-old man with chronic bilateral buttock pain lasting more than 12 months was diagnosed with bilateral piriformis syndrome and right gluteus medius calcific tendinitis with peritrochanteric bursitis. Uniportal endoscopic bilateral piriformis release and right-sided calcific deposit removal were performed in a single session. Clinical discussion: At 3 months, the visual analog scale pain score improved from 8/10 to 0/10, and the Modified Harris Hip Score from 69 to 90. No perioperative complications were observed. Conclusion: This case demonstrates the technical feasibility and favorable short-term outcome of uniportal endoscopic treatment for these coexisting pathologies. Larger studies with longer follow-up are required to evaluate safety, effectiveness, and durability.
