Keterlacakan Pemantauan Terapi Obat dan Mutu Dokumentasi Pelayanan Farmasi Rumah Sakit dalam Seri Kasus Retrospektif

Medication therapy monitoring requires traceable assessment, implementation, and outcomes. This retrospective document review combined a cross-service case series, documentation audit, and neurological charge subanalysis. Twelve PDFs, three Word forms, and one workbook represented 15 episodes. Documentation was analyzed descriptively; charges underwent numeric normalization and quantity–price reconciliation. Median recorded age was 43 years; three cases were pediatric, and all recorded BPJS coverage. Explicit assessments/plans appeared in six cases (40.0%) and PCNE codes in five (33.3%). None of the six assessment reports provided complete linked follow-up. Of 240 charge lines, 224 (93.3%) passed reconciliation and 16 (6.7%) required verification. Reconciled charges totaled IDR3,954,664.65, including IDR260,232.50 for medicines, representing neither complete episode costs nor intervention savings. Findings support standardized follow-up records, counseling responsive to patient barriers, and charge-data integrity. Clinical and economic effects require prospective evaluation.

Abstract

Medication therapy monitoring requires traceable assessment, implementation, and outcomes. This retrospective document review combined a cross-service case series, documentation audit, and neurological charge subanalysis. Twelve PDFs, three Word forms, and one workbook represented 15 episodes. Documentation was analyzed descriptively; charges underwent numeric normalization and quantity–price reconciliation. Median recorded age was 43 years; three cases were pediatric, and all recorded BPJS coverage. Explicit assessments/plans appeared in six cases (40.0%) and PCNE codes in five (33.3%). None of the six assessment reports provided complete linked follow-up. Of 240 charge lines, 224 (93.3%) passed reconciliation and 16 (6.7%) required verification. Reconciled charges totaled IDR3,954,664.65, including IDR260,232.50 for medicines, representing neither complete episode costs nor intervention savings. Findings support standardized follow-up records, counseling responsive to patient barriers, and charge-data integrity. Clinical and economic effects require prospective evaluation.
