Explorative Correlation of Atopic Dermatitis Control Tool with Minimal Disease Activity Patient-reported Outcomes in Patients Treated with Upadacitinib: A Retrospective Real-world Study

Atopic dermatitisis a chronic skin disease that requires long-term monitoring, which can be assessed using Atopic Dermatitis Control Tool (ADCT). Achieving minimal disease activity is also becoming an important issue in the treatment of atopic dermatitis. We aimed to identify which minimal disease sctivity patient-reported outcomes most closely reflect ADCT-defined disease control. We conducted a multicentric, observational retrospective study in patients with moderate-to-severe atopic dermatitis treated with upadacitinib. Outcome measures were collected at different time points (Week 0, 4, 16, 32, 52). Longitudinal changes were assessed with the paired Wilcoxon signedrank test; cross-sectional ADCT-patient-reported outcomes associations were evaluated at each visit using Spearman rank correlation coefficient (ρ). A total of 212 were included. All outcomes improved significantly from Week 4 onwards (all p<0.001). An ADCT score<7 was reached by 84.6% of evaluable patients at Week 4 and 90.1% at Week 52. At each timepoint, ADCT showed the strongest correlation with POEM (ρ range: 0.64-0.64) compared to other minimal disease sctivity patient-reported outcomes. These findings indicate that, among the minimal disease sctivity patient-reported outcomes, POEM most closely reflects the patient's perception of atopic dermatitis control assessed by ADCT. Larger prospective studies are warranted to confirm these preliminary observations.
