Writing and Amending the Clinical Protocol: IND and CTA Lifecycle Strategy for Cardiac Myosin Inhibitors in Obstructive Hypertrophic Cardiomyopathy

Obstructive hypertrophic cardiomyopathy is characterized by excessive cardiac myosin cross-bridge formation that produces left ventricular outflow tract obstruction, exertional symptoms, and a reduced quality of life for affected patients (Ommen et al., 2020). Cardiac myosin inhibitors represent a disease-mechanism-targeted therapeutic class that directly reduces excess actin-myosin cross-bridge formation, distinct from the negative chronotropic and inotropic agents, such as beta-blockers and calcium channel blockers, that have historically constituted first-line pharmacotherapy for this condition. This article is written from the perspective of a clinical trial manager carrying a cardiac myosin inhibitor program from its initial Investigational New Drug (IND) application or Clinical Trial Application (CTA) through subsequent protocol amendments. It reviews the disease and mechanistic rationale, best practices for trial phase and population selection, anticipated risks and pharmacovigilance strategy with particular attention to echocardiographic monitoring for excessive reduction in ejection fraction, statistical sample-size rationale drawn from registered trials, the detailed regulatory requirements and timelines governing the initial submission and amendments across FDA, Health Canada, and EMA frameworks, a representative schedule of assessments spanning a twelve-month period, and the application of artificial intelligence tools to data capture and quality. The article closes with recommendations for regulatory affairs and clinical operations professionals managing this therapeutic category. Keywords: cardiac myosin inhibitor; hypertrophic cardiomyopathy; left ventricular outflow tract obstruction; protocol amendment; Investigational New Drug application; Clinical Trial Application; echocardiographic monitoring; regulatory artificial intelligence
