Once-weekly basal insulin analogs in type 2 diabetes: Pharmacological rationale, clinical evidence, and practical considerations for implementation

Many patients with type 2 diabetes require insulin, but adherence to daily basal regimens is frequently suboptimal because of injection burden, needle anxiety, and lifestyle disruption. Two molecularly distinct once-weekly basal insulin analogs have completed phase 3 evaluation.Insulin icodec achieves prolonged action through fatty diacid-mediated albumin binding (half-life approximately 196 hours), whereas insulin efsitora alfa (half-life 10-14 days) uses immunoglobulin fragment crystallizable-domain (Fc-domain) fusion with neonatal Fc-receptor recycling.Across the six ONWARDS trials, icodec was noninferior to daily comparators and additionally superior in insulin-naïve type 2 diabetes.Across the five QWINT trials, efsitora was non-inferior without demonstrating superiority; QWINT-1 showed that a simplified fixed-step regimen can match titrated daily glargine U100 while reducing the median number of dose adjustments from eight to two. Both agents increased hypoglycemia in type 1 diabetes. The IcoSema coformulation was non-inferior to basal-bolus therapy for glycated hemoglobin and superior for weight, insulin dose, and hypoglycemia, at the cost of more gastrointestinal adverse events. No head-to-head trial has compared icodec with efsitora, and long-term cardiovascular, real-world adherence, and cost-effectiveness data are unavailable.Implementation requires a modified titration cadence, awareness of loading-dose effects, and structured sickday and perioperative planning.
