Writing and Amending the Clinical Protocol: IND and CTA Lifecycle Strategy for Menin Inhibitors in KMT2A-Rearranged Acute Leukemia

Rearrangements of the KMT2A gene (previously known as MLL) define a molecularly distinct, generally high-risk subset of acute leukemia characterized by a dependency on the menin-KMT2A protein interaction to sustain a leukemogenic gene expression program (Issa et al., 2023). Menin inhibitors disrupt this interaction and represent the first targeted therapeutic approach developed specifically for this genetically defined leukemia subset, as well as for NPM1-mutated acute myeloid leukemia, which shares a dependency on the same protein interaction despite a different underlying genetic lesion. This article is written from the perspective of a clinical trial manager carrying a menin inhibitor program from its initial Investigational New Drug (IND) application or Clinical Trial Application (CTA) through subsequent protocol amendments. It reviews the disease and mechanistic rationale, best practices for trial phase and population selection in a relapsed and refractory leukemia population, anticipated risks and pharmacovigilance strategy with particular attention to differentiation syndrome, statistical sample-size rationale drawn from registered trials, the detailed regulatory requirements and timelines governing the initial submission and amendments across FDA, Health Canada, and EMA frameworks, a representative schedule of assessments spanning a six-month intensive monitoring period, and the application of artificial intelligence tools to data capture and quality. The article closes with recommendations for regulatory affairs and clinical operations professionals managing this therapeutic category. Keywords: menin inhibitor; KMT2A rearrangement; acute leukemia; differentiation syndrome; protocol amendment; Investigational New Drug application; Clinical Trial Application; regulatory artificial intelligence
