Elimination of Human Rabies in Goa,India.

Elimination of Human Rabies in Goa, India Description Rabies is a fatal viral zoonotic disease that mainly spreads to humans through the bite or scratch of an infected animal, especially dogs. It affects the central nervous system and is almost always fatal once clinical symptoms appear. However, rabies is preventable through vaccination and timely post-exposure prophylaxis (PEP). Goa, a state in India, implemented an integrated rabies control programme to reduce and eliminate human rabies deaths. The programme combined mass vaccination of dogs, surveillance, public awareness, and improved access to rabies treatment. Programme and Major Interventions The programme was implemented from 2013 to 2019 through collaboration between the Government of Goa, Mission Rabies, and academic partners. It followed a One Health approach, integrating human health, animal health, and community participation. The major activities included: Mass dog vaccination: More than 95,000 dogs were vaccinated, achieving approximately 70% vaccination coverage. Rabies awareness: Schoolchildren, teachers, and community members were educated about dog-bite prevention, wound washing, and the importance of vaccination. Surveillance: A rabies hotline, field investigations, and laboratory testing helped detect suspected rabid animals. Human healthcare: Free post-exposure prophylaxis (PEP) was made available at government medical facilities for people exposed to dog bites. Technology: Smartphone-based mapping and data recording improved vaccination team planning and monitoring. Major Findings Human rabies deaths decreased from 17 in 2014 to zero in 2018 and 2019. Monthly canine rabies cases declined by 92%. The programme demonstrated that eliminating human rabies deaths at the state level was achievable. Nature Communications+1 Significance Goa's experience highlights the importance of sustained dog vaccination, accessible PEP, community education, and coordinated surveillance. It provides a useful public health model for other Indian states aiming to eliminate dog-mediated human rabies. Important note: The study reported zero human rabies deaths in 2018 and 2019; this should not be interpreted as proof of permanent elimination of all rabies transmission. Continued control measures remain essential.
