Analysis of the SIMGOS-Based Electronic Medical Record Completeness Improvement Model at the Outpatient Polyclinic of Dr. Adnaan WD Payakumbuh Regional Hospital

Completeness of Electronic Medical Record (EMR) documentation determines service quality, accreditation, and hospital claims. A 2026 preliminary study at dr. Adnaan WD Payakumbuh Regional Hospital found EMR completeness reached only 60% against a 100% target. This condition contributed to an accreditation downgrade from Paripurna to Utama and Rp500 million in pending claims. This study aimed to analyze the relationship between knowledge, discipline, perceived ease of use, and perceived access speed and the completeness of SIMGOS-based EMR documentation in the outpatient clinic. A sequential explanatory mixed-methods design was used; the quantitative stage was an analytic correlational cross-sectional study of 72 healthcare workers selected by total sampling from March to June 2026. Data were collected using a Likert-scale questionnaire and analyzed with the Spearman test. All respondents (100%) had good knowledge. Spearman tests showed no significant relationship between knowledge (p=0.616), discipline (p=0.687), ease of use (p=0.629), or access speed (p=0.635) and EMR completeness. The four variables do not explain EMR incompleteness. Organizational factors, such as SOPs, supervision, and sanction systems, should be examined further to develop a model for improving EMR completeness.

Abstract

Completeness of Electronic Medical Record (EMR) documentation determines service quality, accreditation, and hospital claims. A 2026 preliminary study at dr.
