Evaluation of Antibiotic Resistance Pattern in Proteus mirabilis Isolates in a Tertiary Care Center in Bangladesh and Detection of Carbapenem and Fosfomycin Resistance Genes among them

Background: Proteus mirabilis, is found to be an emerging threat due to its increasing resistance against numerous antibiotics which tends to limit the available therapeutic options. Resistance against some last resort drugs like carbapenem, fosfomycin is increasing due to the dissemination of plasmids, transposons among bacteria and species. The present study aimed to observe the resistance pattern of P. mirabilis in a tertiary care center in Bangladesh and also to detect carbapenem and fosfomycin resistance genes. Materials and methods: An observational study was conducted in the Department of Microbiology, Dhaka Medical College (DMC) from January 2022 to December 2022. Total 400 different samples were collected from lab and admitted patients. By culture and biochemical tests 30 P. mirabilis were identified and antibiotic susceptibility test was done by disc diffusion method. MIC of imipenem and fosfomycin was done on respective drug resistant strains by agar dilution method. Gene detection was performed by PCR among the resistant strains. Results: Most isolates showed resistance to ceftriaxone (73.33%) ciprofloxacin and aztreonam (70%). A total of 63.33% of isolates were resistant to antibiotics from at least three or more classes and categorized as multi-drug resistant. Among 9 imipenem resistant strains, 33.33% were positive for both bla KPC and blaOXA-48 like genes. 40% isolates were detected positive for fosA3 gene among 10 fosfomycin resistant strains. Conclusion: Current study shows increase in drug resistant strains of P. mirabilis. It also finds that the bla KPC, blaOXA-48 like genes and fosA3 genes are emerging which may act as contributing factor in developing last resort drugs like carbapenem and fosfomycin resistance.
