Integración de la medicina biorreguladora de sistemas como soporte complementario durante la quimioterapia con bleomicina, etopósido y cisplatino en cáncer testicular metastásico: presentación de un caso

AbstractIntroduction: Metastatic testicular germ cell tumors can be curable with cisplatin-based chemotherapy. Evidence regarding systems bioregulatory medicine (SBrM) as support during the BEP chemotherapy regimen is limited. Its incorporation into a case is described, without being able to categorically attribute antitumor efficacy or a causal effect on tolerability to it.Clinical Case: A 34-year-old male with a mixed germ cell tumor of the left testis, pathological category pT2, lymphovascular invasion, retroperitoneal lymphadenopathy, and bilateral lung nodules compatible with metastatic disease. Pre-chemotherapy alpha-fetoprotein (AFP) was 44.4 ng/mL. Clinical stage III was recorded.Intervention: Four cycles of chemotherapy with bleomycin, etoposide, and cisplatin (BEP) were documented, initiated on December 2 and 30, 2024, January 27, and February 24, 2025, with institutional intervals of 28 days. No additional delays, omissions, or dose reductions due to toxicity were recorded. Systems bioregulatory medicine (SBRM) was used before, during, and after chemotherapy as complementary support. Results: AFP decreased to 10.7, 1.13, and 1.00 ng/mL before the second, third, and fourth cycles, respectively, and was 1.63 ng/mL in September 2025. The maximum recorded toxicity was grade 1 according to the CTCAE v5.0 toxicity scale. Functional status remained at 0 on the ECOG scale and between 90% and 100% on the Karnofsky scale. Post-treatment evaluation, performed on May 6, 2025, showed the disappearance of pulmonary and lymph node lesions, with no pathological metabolic activity on the performed PET-CT.Conclusion: Adherence to the institutional schedule, mild toxicity, and a favorable response were observed during BEP treatment with conventional and SBRM support. The single-case design and the modified schedule prevent attributing these results to the complementary intervention or demonstrating the absence of pharmacological interactions.
