Kombinasi Menggambar dan Teknik Menghardik untuk Mengatasi Halusinasi: Studi Kasus
Background: Schizophrenia is a chronic psychotic disorder characterized by disorganized thought, distorted perception, and impaired social functioning. Auditory hallucinations are among its most persistent symptoms and often remain despite antipsychotic treatment, emphasizing the need for complementary psychosocial nursing interventions. Objectives: This case report described changes in auditory hallucination intensity in a patient with schizophrenia following a combined intervention of drawing distraction and a verbal rebuking (menghardik) technique. Methods: A pre-test and post-test case report was conducted over four days involving one 53-year-old female inpatient diagnosed with hebephrenic schizophrenia (F20.1). Hallucination intensity, affective state, and clinical signs were assessed using the Auditory Hallucinations Rating Scale, Emotion Thermometer, and a hallucination observation checklist following SP 1–4 nursing care stages. Data were analyzed descriptively by comparing pre-test and post-test scores. The patient received pharmacotherapy (risperidone, chlorpromazine, lorazepam) throughout the intervention period. Results: The Auditory Hallucinations Rating Scale score decreased from 30 to 22, while the Emotion Thermometer score improved from 2 (sad) to 3 (neutral). Most hallucination-related signs, including pacing, fear, and sweating, resolved by the fourth day, although intermittent self-talk remained. Conclusions: Reduced AHRS scores and improved affective condition were observed after the combined drawing distraction and rebuking intervention, which was delivered alongside pharmacotherapy. Because this is an uncontrolled single case and the contribution of each component cannot be separated, causal effectiveness cannot be inferred and further controlled studies are needed.
