Third Generation Ultrasound-Assisted Liposuction: A Scoping Review and Evidence Map of Clinical Applications, Outcomes, and Complications

Abstract Background Vibration amplification of sound energy at resonance (VASER) is a third-generation ultrasound-assisted liposuction (3rd UAL) platform used for fat emulsification, fibrotic tissue treatment, superficial sculpting, and high-definition liposuction. Its adoption has outpaced the quality of comparative evidence. Objectives To critically evaluate the clinical applications, outcomes and complications of 3rd UAL, distiguishing device-specific evidence from outcomes of combined procedures. Methods PubMed/MEDLINE was searched from inception to 9 August 2026 for human clinical studies explicitly identifying VASER or third-generation ultrasound-assisted liposuction. Reference lists were also screened. Studies were mapped by design, indication, comparator, outcomes, and adjunctive procedures. Owing to heterogeneity, findings were narratively synthesized. Results Comparative evidence was sparse. One contralateral randomized trial found greater retraction normalized to aspirated volume and modestly lower aspirate blood content with 3rd UAL than suction-assisted liposuction, but no clinically discernible patient preference at 6 months. A small randomized comparison of two ultrasound-assisted devices found no consistent biomechanical advantage for 3rd UAL. Large 3rd UAL and high-definition series report feasibility and high satisfaction but are predominantly retrospective, arise from expert centres, and often incorporate power-assisted aspiration, excision, fat grafting, or radiofrequency/plasma devices. Relevant adverse events include seroma, transient hyperpigmentation, fibrosis or induration, prolonged oedema, sensory change, contour irregularity, and port-site thermal injury. Conclusions 3rd UAL, VASER specifically, is best supported as a technique-enabling adjunct for fibrotic fat and advanced three-dimensional contouring. Current evidence suggests selective technical advantages, not universal superiority over conventional liposuction or reliable correction of clinically important skin excess. Future studies should use contemporary comparators, standardized energy reporting, objective skin measurements, validated patient-reported outcomes, and independent multicentre designs.
