Global Trends and Research Hotspots in Digital Health Interventions for Antihypertensive Medication Adherence: A Bibliometric Analysis

Objective: This study aimed to map the global research landscape of digital health and telemedicine in hypertension management by examining publication trends, research contributors, collaboration patterns, intellectual foundations, and emerging research themes. Methods: A bibliometric analysis was conducted using Scopus-indexed publications from 1997 to 2026. The initial search identified 1,068 records, then as many as 819 publications were retained after data cleaning and eligibility screening. Bibliometric analysis was performed using Biblioshiny and VOSviewer, including performance analysis, co-authorship, country collaboration, citation and co-citation analysis, keyword co-occurrence, and thematic mapping. Results: The 819 publications were distributed across 400 sources and involved 6,199 authors, with 55,694 cited references. Scientific production increased substantially, with an annual growth rate of 14.28% and the highest publication output occurring in 2021. Li X was the most productive author, while BMJ Open and Journal of Medical Internet Research were among the leading sources. The co-authorship network comprised 82 connected authors across seven clusters. The United States, United Kingdom, and China were major collaboration hubs. Thematic analysis identified primary care, cardiovascular disease, and stroke as major motor themes, while hypertension, blood pressure, and telemedicine represented fundamental themes. mHealth, mobile health, and self-management showed increasing conceptual importance, whereas medication adherence remained a relatively specialized theme. Conclusion: Global research has increasingly shifted toward integrated digital health, telemedicine, mHealth, and self-management approaches in hypertension management, although research remains concentrated in high-resource settings and requires greater development in low- and middle-income countries.
