Analisis Risiko Keselamatan Kerja pada Aktivitas Pengelasan Menggunakan Metode HIRARC dan Fault Tree Analysis di PT XYZ
This study analyzes occupational safety and health risks in the welding process at PT. XYZ using the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) and Fault Tree Analysis (FTA) methods. Welding activities involve various hazards, including sparks, hot metal fragments, welding fumes, ultraviolet and infrared radiation, high temperatures, and electrical currents. The HIRARC method was applied to identify hazards, assess risk levels based on the AS/NZS 4360:2004 standard, and determine appropriate control measures using the Hierarchy of Control. The results identified ten potential hazards across four groups of work activities, consisting of one high-risk hazard, two moderate-to-high-risk hazards, five moderate-risk hazards, and two low-to-moderate-risk hazards. The highest risk was exposure to hot metal fragments (welding slag), with a risk score of 20 (L = 5; S = 4). Furthermore, Fault Tree Analysis was conducted to determine the root causes of the highest-risk hazard. The analysis revealed that the incident was mainly caused by three factors: failure to use personal protective equipment (PPE), unsafe working conditions, and human error. Based on these findings, risk control recommendations were developed using the 5W+1H approach, including strengthening PPE compliance, installing welding curtains, implementing a permit-to-work system, and conducting regular occupational safety and health training to reduce workplace accidents during welding operations at PT. XYZ.
