A Compliance-First Software Engineering Framework for HIPAA-Regulated Healthcare Systems: Development, Validation, and Implementation

Enterprise software development for U.S. healthcare markets imposes engineering constraints that general-purpose delivery frameworks are not intended to address. Agile, DevSecOps, and Waterfall methodologies consider regulatory compliance an external validation layer, producing systematic late-stage rework, cost overruns, and compliance gaps. This paper introduces the Compliance-First Software Engineering Framework (CF-SEF), a practitioner-derived five-pillar methodology based on over a decade of enterprise software delivery leadership, including the author’s direct responsibility for Andersen’s ISO 27001 certification and the organizational transformation it required. The framework is validated through the ProScan Imaging Platform case study: a large-scale Radiology Information System (RIS) and medical imaging solution delivered for a U.S. healthcare network. Implementation results include a 233% increase in daily order throughput (from 3,000 to 10,000 orders), full HIPAA and ISO 27001 compliance at launch, a delivery timeline approximately 50% shorter than competing vendor estimates, and platform architecture scaled to support 427 U.S. healthcare facilities. The CF-SEF is proposed as a replicable methodology for engineering organizations operating in regulated healthcare environments.
