The Effect of Health Education on the Birth Planning and ComplicationPrevention (P4K) Program on the Knowledge and Attitudes of Primigravida Pregnant Women at Sugiharjo Auxiliary Community Health Center in 2026

Background: The Maternal Mortality Rate (MMR) remains a critical global and national issue. To address this, the government implemented the Birth Planning and Complication Prevention Program (P4K) to improve childbirth preparedness, particularly for first-time mothers (primigravida). At the Sugiharjo Auxiliary Health Center, low adherence to follow-up Antenatal Care (ANC) visits highlights the urgent need for structured educational interventions. Objective: This research focuses on examining how P4K health education affects the understanding and outlook of first-time pregnant women at the Sugiharjo Auxiliary Health Center in 2026. Methods: Implementing a pre-experimental method with a One Group Pretest-Posttest framework, a total of 44 participants were chosen from an overall group of 80 using purposive sampling. The information gathered through structured surveys was examined employing the non-parametric Wilcoxon Signed Rank Test due to the non-normal distribution of the data. Results: Prior to the intervention, most respondents possessed sufficient knowledge (77.3%) but maintained a disagreeing attitude (40.9%). Post-intervention, significant improvements were observed: 59.1% achieved good knowledge, while attitudes shifted positively toward agreeing (36.4%) and strongly agreeing (22.7%). The bivariate Wilcoxon test confirmed these significant changes, yielding p-values of 0.003 for knowledge and 0.002 for attitude (p < 0.05). Conclusion: P4K health education significantly improves knowledge and fosters positive attitudes among primigravida pregnant women. This structured educational approach serves as a strategic tool to enhance mothers' psychophysical readiness, enabling early detection of pregnancy danger signs and timely clinical decision-making.
