Three-dimensional echocardiography for early detection and clinical management of subclinical myocardial injury in metabolic cardiomyopathy
Metabolic cardiomyopathy is commonly linked to insulin resistance, type 1 and type 2 diabetes mellitus, and obesity. It is characterized by early abnormalities, which impair cardiac function before symptoms become evident. The disease often progresses from subclinical diastolic dysfunction to heart failure with preserved and later reduced ejection fraction. Therefore, sensitive diagnostic tools are needed. Three-dimensional echocardiography (3DE), especially when combined with speckle tracking, provides detailed volumetric and deformation analysis. This enables detection of subtle mechanical changes in the myocardium that conventional methods may miss. When 3DE is combined with biochemical markers and advanced imaging modalities, early detection and risk stratification are improved. Moreover, artificial intelligence enhances data collection, analysis, and prediction by automating quantitative assessments and interpreting complex cardiac patterns. Incorporating 3DE into screening and treatment may enable earlier clinical intervention, optimizing care by addressing both metabolic and mechanical dysfunction before irreversible damage develops. This approach aims to preserve myocardial health and improve outcomes.
