Long-term outcomes of mucinous ovarian carcinoma and the association between frailty and survival in older patients

Introduction. Mucinous ovarian carcinoma (MOC) is rare and is associated with poor outcomes at advanced stages. We assessed survival according to frailty, International Federation of Gynecology and Obstetrics (FIGO) stage, grade, and discrepancies between intraoperative and final histopathological diagnoses. Material and methods. This cohort included 87 patients [58 MOC and 29 mucinous borderline ovarian tumors (mBOT)] treated in 2011–2016. In 24 patients aged ≥ 65 years with MOC, frailty was retrospectively assessed using the 11-point Memorial Sloan Kettering Frailty Index (MSK-FI); scores of 1–2 and ≥ 3 were compared. Results. Five- and 10-year overall survival (OS) rates were 43% and 31% in MOC versus 83% and 52% in mBOT. The OS varied by FIGO stage and grade. In 24 older patients with MOC, median OS was 31.0 months with MSK-FI 1–2 and 2.1 months with MSK-FI ≥ 3 (p = 0.018). Frozen-section discordance was not associated with OS (p = 0.122). Conclusions. In this surgically treated cohort, higher FIGO stage and higher tumor grade were associated with shorter OS in patients with MOC. Among patients aged ≥ 65 years, higher frailty was associated with markedly shorter OS. Frozen-section reclassification from mBOT to MOC was not associated with shorter OS.

We assessed survival according to frailty, International Federation of Gynecology and Obstetrics (FIGO) stage, grade, and discrepancies between intraoperative and final histopathological diagnoses.
