Man With Eye Pain, Vision Loss, and Hip Pain
A 47-year-old man without any past medical history presented to the emergency department with 1 day of sudden onset, painful left-sided vision loss. The patient also reported 3 weeks of atraumatic right hip pain. The initial examination was remarkable for a thin, well-appearing man in no acute distress. He was afebrile at triage, but 4 hours later, his temperature was 39.6 °C (103.2 °F). He had no light perception in his left eye. When shining a light into the left eye, the right pupil dilated. The right hip was tender to palpation over the greater trochanter, with full range of motion and positive femoroacetabular impingement.
