[5] Research Report and Forensic Audit Framework: Exposing Illicit Biomedical Interventions, Pharmacological Abuse, and Organ Harvesting Pathways in Intensive Care Units

This technical monograph establishes a comprehensive forensic and clinical audit framework to monitor, expose, and legally challenge illicit violations in Intensive Care Units (ICUs), specifically focusing on the abnormal transition from severe traumatic brain injury (TBI) treatment to organ procurement pathways without fully informed consent. By deconstructing the 7-step clinical sequence—from emergency triage loopholes to post-harvest evidence obfuscation—this study rigorously contrasts legitimate neuro-resuscitation protocols (utilizing Mannitol, Hypertonic Saline, Levetiracetam) against abusive pharmacological patterns. The latter is characterized by continuous high-dose infusions of neuromuscular blocking agents (Rocuronium, Vecuronium, Cisatracurium, Succinylcholine) and high-dose sedatives (Propofol, Midazolam, Thiopental, Fentanyl) in the absence of Bispectral Index (BIS) depth-of-anesthesia monitoring. Furthermore, this report defines objective quantitative audit metrics, including input-output discrepancy formulas, verification of the absence of tapering logs, and hardware-level black box log extraction from infusion pumps. Underpinned by international legal frameworks such as the Oviedo Convention, the Rome Statute of the International Criminal Court (ICC), and emergency intervention mechanisms of INTERPOL, this study provides an actionable family defense manual and a cross-border investigative activation protocol to safeguard human rights and enforce medical legal accountability. Systemic Overload as a Driver of Clinical Errors and Legal Exposure: It is essential to clarify that objective clinical and bioethical standards—such as brain death criteria and organ donation frameworks—are intrinsically rooted in medical progress and humanitarian intent. However, when healthcare institutions operate under severe structural capacity overload and rigid financial self-autonomy mandates, operational compression and audit-anxiety significantly escalate the risk of systemic clinical errors, diagnostic compromises, and severe legal exposure for all stakeholders involved. For frontline practitioners, chronic systemic strain increases operational vulnerability and administrative non-compliance. For patient families, intense psychological pressure at the bedside risks inducing premature, legally disadvantageous decisions. Rather than attributing malicious intent to individuals, this study dissects these structural pathologies to provide an objective framework for risk mitigation, evidence preservation, and strict legal compliance in high-pressure clinical environments.
