Aplikasi Temuan Radiografi Dada dan Computed Tomography pada Tropical Pulmonary Eosinophilia (TPE) dalam Triase Keperawatan Gawat Darurat: Sebuah Tinjauan Sistematis
Tropical Pulmonary Eosinophilia (TPE) is a severe and relatively rare clinical manifestation of pulmonary hypersensitivity caused by parasitic filarial nematode infections, primarily *Wuchereria bancrofti* and *Brugia malayi*. Although characteristically marked by paroxysmal nocturnal cough, extreme absolute peripheral blood eosinophilia (>3,000 cells/μL), and massively elevated total serum IgE levels (>1,000 IU/mL), radiological findings on chest radiography (CXR) and high-resolution computed tomography (HRCT) often display a wide spectrum of variations. These range from classic reticulonodular patterns to atypical lesions such as complex cavities, ground-glass opacities (GGO), and endobronchial mucosal involvement. This study aims to conduct a comprehensive, systematic observation of chest radiographic and CT scan appearances in TPE patients and to map the matrix of their radiological differential diagnoses. The objective was to identify chest radiographic and computed tomography findings associated with TPE through a systematic review of articles published within the last decade in databases including PubMed, Embase, Web of Science, ScienceDirect, and the Cochrane Central Register of Controlled Trials. A literature search yielded 12 articles that were analyzed following PRISMA screening protocols. The study provides a summary of TPE, highlighting the dynamic spectrum of radiological appearances on CXR and HRCT. Findings include normal CXR results despite endobronchial involvement, as well as instances of pleural effusion. Chest HRCT demonstrated significantly higher sensitivity and specificity than CXR in identifying early parenchymal involvement and distinguishing TPE from other disease entities that mimic its presentation.
