Intraocular pressure changes post primary and non-primary retinectomy

Background To report intraocular pressure (IOP) changes after non-primary (NPR) and primary retinectomy (PR), to assess a correlation between final IOP and the length of retinectomy. Methods Retrospective cohort study: 99 consecutive patients who underwent retinectomies for rhegmatogenous retinal detachment complicated by proliferative vitreoretinopathy between 2014–2021. Results Mean final IOP was 13.79 ± 6.60 and 15.18 ± 9.62 mmHg among NPRs and PRs respectively, the difference with mean preoperative IOPs (16.64 ± 8.99 mmHg and 12.22 ± 4.54 mmHg among NPRs and PRs respectively) was significant among NPRs (p = 0.013). No differences were found between mean final IOP of eyes without SO and with SO in situ in both groups (p = 0.333, p = 0.123). The mean retinectomy extent was 5.06 ± 2.11 clock hours among NPRs and 7.36 ± 3.28 clock hours among PRs, the difference was significant (p < 0.001) . A significant inverse correlation was found between retinectomy length and final IOP in cases which underwent successful removal of silicone oil (ROSO) post-retinectomy (p = 0.038), in cases with oil in situ at last follow-up (p = 0.035) and among NPRs (p = 0.022). Conclusion final IOP significantly diminished after NPR. Retinectomy length was shorter among NPRs, and significantly inversely correlated with final IOP in cases that underwent successful ROSO, with oil in situ at final follow-up and in NPRs.
