SGLT2 Inhibitor Utilization in Patients with Type 2 Diabetes and Chronic Kidney Disease in a Community Health Center
Abstract: This study evaluated sodium-glucose cotransporter 2 inhibitor (SGLT2i) prescribing and adherence in patients with type 2 diabetes (T2DM) and chronic kidney disease (CKD) treated at a federally qualified health center (FQHC). Methods. This cross-sectional study included patients with T2DM and CKD who received care from the FQHC between January 1, 2020 – December 31, 2023. The proportion with a prescription order for an SGLT2i was identified. Medication adherence was estimated for patients filling their SGLT2i prescription at the FQHC pharmacy using an average proportion of days covered (aPDC). Results. Of 885 included patients, 48% were prescribed an SGLT2i during the study period. Mean aPDC for the subset (n=277) filling their SGLT2i at the FQHC pharmacy was 86%; 70% had an aPDC of 80% or greater. Conclusion. Prescribing for SGLT2i for patients with T2DM and CKD at an urban FQHC, while not optimal, was higher than described in other studies. Adherence was high for the patients using the FQHC's pharmacy.
