Left Ventricular Noncompaction Cardiomyopathy and Thromboembolic Complications: A Case Report and Focused Review of the Literature

Left ventricular noncompaction cardiomyopathy (LVNC) is a rare myocardial disorder characterized by prominent trabeculations and deep intertrabecular recesses. Thromboembolic complications represent a major source of morbidity and mortality, yet optimal risk stratification and anticoagulation strategies remain poorly defined. The authors have reported the case of a patient with previously undiagnosed LVNC who presented with rapidly progressive heart failure and developed acute, recurrent cardioembolic strokes. Cardiac imaging revealed extensive left ventricular trabeculations, a severely reduced ejection fraction, and bilateral atrial appendage thrombi. Despite systemic anticoagulation, the patient experienced rapid respiratory and hemodynamic decompensation resulting in death. LVNC is associated with a substantial chance of thromboembolic events, particularly in patients with ventricular dysfunction. This case highlighted the crucial importance of early recognition and the need for individualized anticoagulation strategies in patients at high risk.
