Cheilitis Granulomatosa Managed with Kenalog in Orabase and Tantum Verde: A Case Report

Cheilitis granulomatosa (CG), also known as Miescher cheilitis, is a rare inflammatory condition characterized by persistent or recurrent swelling of one or both lips. Its etiology remains unclear, with possible associations involving genetic predisposition, allergic reactions, immune-mediated mechanisms, and infectious factors. This report describes the clinical presentation and management of CG in a 15-year-old female who presented to the Department of Oral Medicine, Faculty of Dentistry, University of Zawia, with a five-day history of sudden-onset lower lip swelling associated with tenderness, burning sensation, and erythema. Clinical examination revealed diffuse, soft, non-pitting swelling with well-defined margins and erythematous mucosa accompanied by fissures. A provisional diagnosis of cheilitis granulomatosa was established based on the clinical findings. The patient was managed using topical triamcinolone acetonide 0.1% dental paste (Kenalog in Orabase) in combination with Tantum Verde spray (benzydamine hydrochloride). Following 10 days of treatment, complete resolution of the lip swelling and associated symptoms was observed, with no adverse effects reported. This case demonstrates a favorable clinical response to a combination of topical anti-inflammatory preparations in the management of localized cheilitis granulomatosa. Although the observed improvement is encouraging, further clinical studies are required to establish the effectiveness and long-term outcomes of this therapeutic approach.
