Minimally invasive simultaneous operations on the lungs and abdominal organs for secondary tumor lesions

Removal of metastatic foci using minimally invasive technologies for oligometastatic lesions of the lungs and other organs helps improve long-term treatment outcomes. Objective. Analysis of the results of simultaneous surgical treatment in patients with oligometastases in the lungs using minimally invasive technologies. Material and methods. We retrospectively analyzed the treatment outcome of 9 patients who underwent simultaneous surgical procedures using a minimally invasive approach: lung and liver resection in 3 patients with colorectal cancer (CRC), colon and lung resection in 4 patients with CRC, liver and lung resection in a patient with uveal melanoma, and removal of a pelvic tumor and lung resection in a patient with endometrial cancer. Results. There were no severe postoperative complications or deaths. The median follow-up was 27 months. During the observation period, four of the seven patients with colorectal cancer were diagnosed with lung metastases, and one with brain metastases. Two patients with colorectal cancer showed no signs of disease progression at the time of analysis. A patient with endometrial cancer developed a locoregional recurrence in the pelvis, which required surgical intervention. A patient with uveal melanoma underwent stereotactic radiation therapy after disease progression (lung metastasis), which resulted in stabilization of the disease. During the observation period, one patient with colorectal cancer died due to brain metastases. Conclusion. The use of minimally invasive technologies improves the safety of simultaneous surgeries on the lungs and abdominal or pelvic organs affected by tumors and may potentially expand the indications for such procedures.
