Computed tomographic‐guided surgical repair of third tarsal bone slab fractures in Thoroughbred racehorses

Summary Background Slab fractures of the third tarsal bone are relatively common injuries in racehorses. Surgical repair guided by radiographic imaging has been reported; however, 2D imaging may limit accuracy in screw placement. Computed tomographic (CT) imaging has the potential to improve surgical placement of screws, and this may contribute to enhanced case management. Objectives (i) To describe a novel technique for CT‐guided repair of slab fractures of the third tarsal bone and (ii) to evaluate technical feasibility and accuracy of implant placement from a series of clinical cases, including case outcomes. Study design Retrospective case series (2023–2025). Methods CT‐guided surgical repair of slab fractures of the third tarsal bone was performed. Case records and CT images were reviewed. Follow‐up information was obtained from referring veterinarians, race records and owners/trainers. Results Eight cases underwent CT‐guided screw repair. Fractures in seven cases were repaired with a single screw, with accurate positioning achieved. In one case, two screws were used for repair as the first screw was positioned eccentrically in the fracture fragment. All horses returned to training after surgery and six raced. Main limitations The small number of cases and lack of a cohort of control horses limit conclusions on the contribution of surgery to case management. Conclusions CT‐guided repair of slab fractures of the third tarsal bone can be reliably performed, positioning screws accurately within the central third of fracture fragments and avoiding penetration of articular surfaces.
