Design Of Manual Medical Record Media Transferto Electronic Medical Records At Home Sickness Class II 03.05.01 Dustira

Digital transformation in healthcare services encourages hospitals to improve medical record management from manual systems to Electronic Medical Records (EMR). Manual medical record management presents several challenges, including storage requirements, document damage risks, and time-consuming processes for retrieving and providing information. Therefore, a structured media conversion process is needed to support digital medical record management. This study aims to design a system for converting manual medical records into EMR at Tk. II 03.05.01 Dustira Hospital. The study applies the Waterfall system development method, consisting of requirements analysis, system design, implementation, and testing. Data were collected through observation, interviews, documentation, and literature review to identify system requirements and understand the existing medical record management process. The results produce a media conversion system design covering patient data management, document scanning and uploading, digital storage, medical record retrieval, and user access management. The proposed system is expected to improve management effectiveness, accelerate information retrieval, and reduce dependence on physical documents. This study contributes a system design that can serve as a foundation for implementing manual medical record conversion toward more structured EMR management.
