Risk prediction for locoregional recurrence in mutant‑negative unresectable stage III non‑small cell lung cancer after concurrent chemoradiotherapy: A retrospective study

Locoregional recurrence (LRR) is a major factor affecting prognosis in patients with driver mutation-negative unresectable stage III non-small cell lung cancer (NSCLC) after concurrent chemoradiotherapy. The present study aimed to identify independent risk factors for LRR in this patient population and develop a simple predictive model. The present retrospective study included 134 patients with NSCLC treated between February 2020 and April 2025.Clinicopathological factors were analyzed using Cox regression, and notable predictors were integrated into a risk stratification model.Multivariate analysis identified T3-4 stage and persistently abnormal cytokeratin fragment antigen 21-1 levels after treatment as independent risk factors for LRR.A risk stratification model incorporating these two factors successfully stratified patients into low-and high-risk groups, with marked differences in time to recurrence. The findings of the present study indicate that highrisk patients can benefit from more intensive local treatment or closer surveillance to improve tumor control and clinical outcomes.
