Recent Developments and Clinical State of Regenerative Alloplastic Bone Grafts in Craniofacial Reconstruction

Alloplastic bone grafting has become one of the fastest-moving areas of research in craniomaxillofacial reconstruction. Over the past decade, the field has expanded from a small set of biocompatible fillers into a broad design space encompassing calcium phosphate ceramics, bioactive glass, resorbable polymers, hybrid composite scaffolds, and custom three-dimensionally printed constructs. This is further accelerated by a large body of work on ionic doping, antimicrobial modification, and biological payloads such as bone morphogenetic protein and stem cell-derived exosomes. This pace, however, has not been matched clinically. Current practice still rests largely on hydroxyapatite cements, demineralized bone matrices, and inert patient-specific implants, while functionalized scaffolds that dominate the literature remain confined to bench-top and small-animal models. This article reviews the materials and design principles that define modern regenerative alloplastic grafts, covering the fundamentals of bioceramics, composites, scaffold architecture and manufacturing, and biological augmentation. It then examines the current state of clinical applications of regenerative grafts across craniomaxillofacial reconstruction relative to other alloplastic options, and concludes with frontier developments expected to define the future of the field.
