Meconium Periorchitis: Clinical Observations

Objective . To demonstrate the ultrasound semiotics of meconium periorchitis (MP) in infants and to substantiate the management tactics. Materials and Methods. Two clinical cases of MP were analyzed. Data from ultrasound, colour Doppler mapping, MRI (in the second case), intraoperative and pathomorphological verification were used. Results . In the first case, MP manifested as hydrocele with multiple calcifications (twinkling artifact), which allowed for a conservative management approach. A two-year follow-up confirmed a benign course. In the second case, an encysted paratesticular mass formed, requiring differential diagnosis with teratoma. After MRI, a scrotal exploration and mass excision were performed. Histology confirmed organized meconium masses. In both cases, the testes were preserved. Conclusion . MP is a rare pathology that should be included in the differential diagnosis of scrotal masses in infants. Ultrasound with CDM can identify characteristic signs and, in typical cases, allows for a conservative approach, avoiding unnecessary surgery.
