Hydroxychloroquine-Induced Hyperpigmentation in a Woman with Systemic Lupus Erythematosus: A Rare Complication in a Dark-Skinned Phenotype

ABSTRACTHydroxychloroquine (HCQ) is frequently used in the treatment of systemic lupus erythematosus (SLE) for its anti-inflammatory and immunomodulatory properties. However, it can cause dyschromia, a source of aesthetic concern and anxiety for patients. We report the case of a 32-year-old female with a dark phenotype, diagnosed with SLE according to the ACR/EULAR 2019 classification criteria, who developed hyperpigmentation during HCQ therapy. The hyperpigmentation presented as confluent brown-gray macules forming patches on the face and scalp, appearing approximately 14 months after resuming treatment at a dose of 400 mg/day. This iatrogenic complication is rarely reported in dark-skinned phenotypes.

This iatrogenic complication is rarely reported in dark-skinned phenotypes.
