Analysis Of Outpatient Data Entry Processes On The Quality Of Medical Record Services At Hermina Arcamanik General Hospital

This study aimed to analyze outpatient data entry processes and identify factors and obstacles related to medical record service quality at RSU Hermina Arcamanik. A descriptive qualitative method involved five informants: three outpatient registration officers, one medical records officer, and one outpatient registration coordinator. Data were collected through observation, semi-structured interviews, and documentation, then analyzed using the Miles and Huberman model. Data validity was assessed through technical triangulation. The findings showed that data entry involved patient identification, medical record number searches, data entry, identity and payment-guarantee verification, and directing patients to subsequent services. Data completeness and accuracy were maintained through identity matching and re-verification, although missing identity documents and duplicate medical record numbers remained issues. Obstacles included staffing constraints, SIMRS delays, difficulties with digital registration, biometric verification problems, and payment-guarantee confirmation. Accurate data entry and efficient service processes are essential to supporting medical record service quality.
