Strengthening mental health care access by addressing staffing challenges at rural health facilities in the Northern Cape province of South Africa: a socio-ecological perspective

Objective: Rural mental health services in South Africa face persistent staffing challenges. Using a socio-ecological perspective, the study explored how health workers and managers in two Northern Cape districts understood these challenges and proposed strategies for improving access to care.Method: The study involved virtual, in-depth interviews with 12 purposively selected health professionals and managers between October and December 2024. Interview transcripts were analyzed thematically, and the findings were organised across socio-ecological levels. Results: Participants linked workforce shortages to high caseloads, limited professional development, difficult living and working conditions, fragmented primary care integration, delayed recruitment and inadequate resources. They proposed better supervision and retention support, carefully supported task-sharing, community partnerships, rural training pathways and remote specialist support. These options could also create new challenges, as giving more tasks to already stretched staff may increase burnout, while digital services may leave out people with poor connectivity or limited access to data. Conclusion: Recruitment and retention depend on decisions across the health system, including how posts are funded and filled, how primary care teams are supported, and whether proposed service changes can work in remote communities. More research is needed to establish whether the proposed solutions would be effective.

They proposed better supervision and retention support, carefully supported task-sharing, community partnerships, rural training pathways and remote specialist support. More research is needed to establish whether the proposed solutions would be effective.
