Intestinal and Urinary Helminthiases: Prevalence, Polyparasitism and Associated Factors among Schoolchildren in the Sahelian and Sudanian Zones of Chad

ABSTRACTIntroduction. Polyparasitism involving intestinal helminths and schistosomes is common among schoolchildren in sub-Saharan Africa, but its structure and determinants remain poorly documented in Chad. This study aimed to describe helminth species richness, infection intensity, infracommunity structure, and factors associated with polyparasitism among schoolchildren in the Sahelian and Sudanian zones of Chad. Methods. A cross-sectional survey was conducted in 19 rural public schools between September 2021 and February 2022. Stool samples were examined using a single Kato–Katz smear read twice, and urine samples were examined after centrifugation. Factors associated with polyparasitism were assessed using multivariable logistic regression. Results. Of the 1,443 pupils enrolled, 1,408 provided both stool and urine samples. Nine helminth taxa were identified. The prevalence of intestinal helminths was 35.87% (505/1,408), while the prevalence of Schistosoma haematobium was 45.46% (656/1,443). Heavy-intensity infections affected 41.6% (273/656) of S. haematobium carriers and 42.1% (83/197) of S. mansoni carriers. Polyparasitism affected 23.08% (325/1,408) of pupils and comprised 28 infracommunity types. Polyparasitism was less likely in the Sahelian zone than in the Sudanian zone (adjusted odds ratio [aOR], 0.56; 95% confidence interval [CI], 0.42–0.74) and more likely among pupils aged 15–18 years than among those aged 10–14 years (aOR, 1.45; 95% CI, 1.03–2.06). Conclusion. Chadian schoolchildren harbour a diverse helminth fauna, with frequent polyparasitism and heavy schistosome infections. An integrated control strategy combining praziquantel and albendazole, health education, and improved sanitation is needed.RÉSUMÉ Introduction. Conclusion.
