An A-to-Z Regulatory Journey of a Class IV Transcatheter Aortic Heart Valve Under Health Canada
This article traces the generalized A-to-Z regulatory journey of a representative Class IV transcatheter aortic heart valve, using Canada as the lead jurisdiction, from device description and classification through licensing, market access, and post-market surveillance. The article examines the competitive and reimbursement landscape for this device category, compares the US Premarket Approval pathway, and shows how International Medical Device Regulators Forum guidance, the Medical Device Single Audit Program, and a single harmonized pivotal trial can serve both regulators efficiently. All design assumptions discussed are modelled on the published characteristics of marketed transcatheter heart valves generally and on the clinical evidence cited throughout, rather than describing any specific real company's product. The article closes with recommendations for regulatory affairs professionals pursuing a Canada-first strategy for a Class IV structural heart device. Keywords: transcatheter aortic valve replacement; Class IV medical device; Health Canada; Premarket Approval; IMDRF; MDSAP; regulatory artificial intelligence
