Real-World Use of Brentuximab Vedotin in Adult Patients With Classic Hodgkin Lymphoma: A Retrospective Analysis From a Colombian Healthcare Registry (2016-2023)

BackgroundBrentuximab vedotin (BV), an anti-CD30 antibody-drug conjugate, is approved for the treatment of classic Hodgkin lymphoma (cHL) in various settings, including relapsed/refractory disease (R/R), post-transplant consolidation, and more recently, as part of first-line therapy in combination with chemotherapy for advanced-stage disease. However, data from Latin American real-world settings remain limited. ObjectiveThe study aimed to evaluate treatment patterns, effectiveness, and safety of BV in adult patients with cHL in Colombia. MethodsWe conducted a retrospective cohort study using both structured and unstructured data from electronic health records of a national insurer.Adults (≥18 years) with cHL treated with BV between 2016 and 2023 were included. The primary endpoints were progression-free survival (PFS), overall survival (OS), and objective response rate (ORR).Selected adverse events (AEs) were also assessed. ResultsSeventy-three patients were included (median age: 30.8 years; sex: 61.6% (45/73) male).BV was administered as monotherapy (35/73; 47.9%) or in combination (38/73; 52.1%), mainly in the refractory (salvage) setting (41/73; 56.2%), or as post-ASCT consolidation (17/73; 23.3%) and in a smaller proportion as first-line treatment (6/73; 8.2%).Median PFS in R/R patients was 19.5 months (95% CI, 12.83-28.66),and median OS was not reached during the follow-up.ORR was 52.1% (CR: 27.4%; PR: 24.7%) with higher CR rates in combination therapy (12/38; 31.6%)versus monotherapy (8/35; 22.9%).Peripheral neuropathy occurred in 8.2% (6/73) of patients. ConclusionIn this national cohort, BV showed sustained effectiveness and tolerability across multiple treatment settings, particularly in post-ASCT consolidation or other late-line settings.Outcomes align with international real-world data, reinforcing BV's role in the therapeutic landscape of cHL in resource-variable environments.
