From Breakthrough Designation to Bilateral Approval: A Regulatory and Project Management Reconstruction of a Pulsed Field Ablation System
This article reconstructs the generalized A-to-Z regulatory journey of a representative pulsed field ablation (PFA) system, a cardiac ablation platform for atrial fibrillation (AF) treatment that has received regulatory approval in both the United States and Canada. The analysis proceeds through device description, classification, regulatory strategy, market analysis, global strategy, project management and risk, and integration and quality. A device of this kind applies irreversible electroporation to cardiac tissue, a mechanism distinct from the thermal energy sources used in conventional radiofrequency and cryoballoon ablation, offering a potentially more favourable collateral-injury profile and a faster procedural workflow. The article develops the classification rationale in both jurisdictions, the pivotal clinical evidence and real-world registry data supporting approval, the rapid market adoption dynamics typical of a genuinely novel energy source entering an established therapeutic category, and the multi-mechanism global regulatory reuse strategy that allowed near-simultaneous authorization across several jurisdictions. The article closes with recommendations for regulatory affairs professionals managing novel-energy-source device programs in a crowded, well-established therapeutic category. Keywords: pulsed field ablation; atrial fibrillation; Class III/IV medical device; Premarket Approval; Health Canada; irreversible electroporation; regulatory artificial intelligence
