Tirzepatide Use Resulted in Clinically Meaningful Weight Reduction in Individuals With Obesity or Overweight: 12‐Month Real‐World Results

AIMS: This study assessed the real-world effectiveness of tirzepatide in commercially insured individuals without diabetes, eligible for obesity medications (OMs), including glucagon like-peptide-1 receptor agonist (GLP-1 RA)-naïve individuals (i.e., no GLP-1 RA prescription during the 12-month pre-index period) and GLP-1 RA-experienced individuals. METHODS: This retrospective, observational cohort study used Optum's de-identified Market Clarity Data (Optum Market Clarity). Adults with ≥ 1 prescription claim for tirzepatide between November 2023 and March 2024, with continuous medical/pharmacy enrollment for ≥ 12 months pre-index and post-index and available pre- and post-index data were included. Baseline demographics, clinical characteristics, and treatment effectiveness (change from baseline in weight, body mass index, and cardiometabolic risk factors) were assessed among OM-eligible individuals who were persistent on tirzepatide for ≥ 12 months. RESULTS: The GLP-1 RA-naïve and experienced cohorts included 230 individuals (mean age: 49.5 years, 72.2% female) and 123 individuals (mean age: 48.9 years, 81.3% female), respectively, who were persistent on tirzepatide for 12 months with documented pre- and post-index weight measurements. At baseline, more than 85% of individuals in each cohort had ≥ 1 obesity-related complication, most commonly dyslipidemia, hypertension, and prediabetes. At 12 months post-index, the GLP-1 RA-naïve cohort achieved a mean weight reduction of 19.3%, while the GLP-1 RA-experienced cohort achieved a mean weight reduction of 13.1%. CONCLUSIONS: After 12 months of persistent tirzepatide use, individuals experienced clinically meaningful weight reduction, comparable to outcomes reported in clinical trials. These real-world findings were observed both in GLP-1 RA-naïve and experienced individuals.
