Integrated Evidence, Tiered Risk: A Regulatory Strategy for a Dual-Chamber Leadless Pacemaker System
A sponsor extending an approved leadless pacing platform from single-chamber to dual-chamber pacing must decide how to allocate evidence-generation resources when some functions are new and others are inherited from the earlier platform. This article addresses that problem for a representative dual-chamber leadless pacemaker system, synchronized beat by beat through implant-to-implant communication between two separately implanted devices. The article develops a classification rationale for both Canada and the United States, a regulatory strategy built around a single integrated evidence program filed in parallel to both regulators, a clinical evidence summary, a strategic and market analysis addressing both the direct single-chamber competitor and a broader shift toward alternative pacing approaches, a global regulatory comparison including a cautionary historical counterexample from an earlier leadless pacemaker development program, and a risk-tiered project management framework. The article closes with recommendations for regulatory affairs professionals managing platform-extension strategy for implantable cardiac devices. Keywords: dual-chamber leadless pacemaker; Class IV medical device; Health Canada; premarket approval; regulatory strategy; post-market surveillance; regulatory artificial intelligence
