Regulatory Strategy and Project Management Across the Development Lifecycle of a Transcatheter Tricuspid Valve Replacement System: A Total-Product-Lifecycle Case Study in Class III/Class IV Structural Heart Technology
A representative transcatheter tricuspid valve replacement system illustrates a high-risk structural-heart technology that enables replacement of the native tricuspid valve without conventional open-heart surgery. A system of this kind typically integrates a bovine-pericardial valve, a self-expanding nitinol frame and anchoring architecture, and a transfemoral venous delivery platform. Such devices have received Breakthrough Device designation and subsequent Premarket Approval (PMA) in the United States for patients with symptomatic severe tricuspid regurgitation (TR) despite optimal medical therapy when replacement is deemed appropriate by a heart team. This article reconstructs the generalized regulatory journey for this device category and integrates classification, standards, clinical evidence, market positioning, global harmonization, project management, regulatory risk, human factors, quality systems, and real-world evidence. An illustrative Gantt chart provides a realistic development model for a device of this risk class. All budgets, schedules, and risk ratings presented are illustrative consultancy assumptions and are not drawn from any real company's internal records. Keywords: transcatheter tricuspid valve replacement; structural heart; Class III/IV medical device; Premarket Approval; Health Canada Class IV; project management; regulatory artificial intelligence
