Regulatory Strategy and Project Management for a Next-Generation AV-Synchronous Leadless Pacemaker: A Change-Assessment and Dual-Jurisdiction Case Analysis

This article examines the regulatory strategy and project-management considerations associated with a representative next-generation leadless pacemaker offering atrioventricular (AV) synchrony through accelerometer-based mechanical sensing, a high-risk implantable cardiac pacing system extending an earlier single-chamber leadless platform. The article reconstructs a generalized regulatory journey for a device of this kind and applies it to a strategic regulatory-planning analysis centred on the change-assessment process required when a sponsor seeks approval for a next-generation device building on a previously approved platform. The article develops the classification rationale in both the United States and Canada, a requirements-matrix approach for coordinating dual-jurisdiction submissions, a SWOT-based market analysis, and an integrated project management and risk framework, closing with recommendations for regulatory affairs professionals managing platform-generation device programs. Keywords: leadless pacemaker; AV synchrony; medical device regulation; Premarket Approval supplement; Health Canada; change assessment; regulatory artificial intelligence
