Regulatory Strategy and Project Management Across the Development Lifecycle of a Dual-Chamber Leadless Pacemaker System: Closed-Loop Classification and Post-Market Cybersecurity Risk

Leadless pacemakers remove the two parts of a conventional pacing system that cause most of its problems: the chest pocket and the transvenous lead. This article follows the regulatory journey of a representative dual-chamber leadless pacemaker system, chosen for three reasons: it is a permanent cardiac implant falling in the highest risk class in both Canada and the United States, it reached market recently enough that its regulatory milestones are well documented in public sources, and it illustrates a closed-loop device classification question distinct from a conventional single-device implant. The article follows a seven-part structure: device description and innovation, classification rationale, regulatory strategy, strategic and market analysis, global regulatory analysis, project management and risk, and integration and quality, with particular attention to post-market cybersecurity risk as the single most consequential ongoing risk category for a device of this kind. Keywords: dual-chamber leadless pacemaker; closed-loop system; Class IV medical device; cybersecurity; premarket approval; regulatory strategy; regulatory artificial intelligence
