Suicide Prevention in Patients With Schizophrenia Spectrum Disorders: Assessment and Integrated Interventions

Individuals with schizophrenia spectrum disorders have a higher risk of suicide than the general population. Suicide prevention is central to care for people with schizophrenia spectrum disorders. This narrative review integrates evidence on high-risk periods, therapeutic assessment, pharmacotherapy, psychosocial interventions, and continuity of care, with attention to implementation in Korea. Assessment should connect historical vulnerability, current modifiable factors, foreseeable changes, and available protective resources in an individualized formulation rather than rely on risk categories to predict suicide. Early illness and the period immediately after discharge warrant particular attention. Integrated care combines optimized pharmacotherapy, suicide-focused psychosocial interventions, early intervention services, and sustained case management. Evidence for improvements in suicidal ideation should be distinguished from evidence for reductions in attempts or deaths, and findings from other populations require cautious application. Hospital and community services should jointly plan transitions, confirm timely follow-up, and clarify responsibilities. Strengthening these arrangements, staff support, and implementation research may help translate assessment into sustained, coordinated care.
